Click on the 5th Button "Correction of entries in electoral roll" or click here . A FORM-08 will be opened in your browser. In that page you have to enter the details about your previous VOTER ID and you have to choose the option, which you want to change in your new Voter ID. FORM-08 has six parts.

 PART-01

Here you have to choose the state and Assembly/Parliamentary Constituency

 PART-02 
Here you have to enter the Applicant's details  like
  1. Name 
  2. Surname 
  3. Part number of electoral roll 
  4. Serial No. of Electoral Roll
  5. Elector's Photo Identity Card Number (if issued)

Here you have to choose the option or particulars which you want to change in your OLD Voter ID
  1. Name
  2. Photograph 
  3. EPIC 
  4. Address 
  5. Date of Birth 
  6. Name of Relative 
  7. Type of Relation
  8. Gender


 PART-04 

This part depend on the PART-03. The option you want chosen in the previous part will be shown here. You have to enter the correct details which you want to see in your NEW VOTER ID

Here you have to upload the 
Supporting Document. All the image size should be in 100kb or less.

Here you have to enter your email id and mobile number, and you have to made a declaration.
